Agape Healthcare Achieves National Recognition Through 'Quality Connections' Program

The National Hospice and Palliative Care Organization (NHPCO) recognized Agape Healthcare for earning four rings in its Quality Connections (QC) program for 2023. Quality Connections is the only national program designed to support hospice and palliative care providers’ delivery of high-quality, person-centered care. Agape completed this program by participating in four program pillars, represented by rings: Education, Application, Measurement, and Innovation. Participants earn a ring when they complete four activities in a program pillar in a calendar year.

Completing some of these pillars can take over a year. Agape Healthcare achieved all four rings in under a year, and we are one of only two hospice companies in Colorado with this recognition! We are proud of our team’s achievement and commitment to continuous quality improvement.
